Job Summary:
We are seeking aCLINICAL NURSE IIin ourDermatology Departmentlocated inDurham, NCindependently plans and provides professional nursing care for patients in the Dermatology ambulatory clinic setting in accordance with the medical and nursing plans of care and established policies and procedures. He/she provides nursing services to patients and families in accordance with the scope of the RN as defined by the North Carolina Board of Nursing.
Job Duties and Responsibilities:
- Assess and facilitate coordination of multidisciplinary care across the care continuum. Provide communication, advocacy, and education in a culturally competent manner.
- Ability to clinically triage new patient referrals efficiently, to ensure timely access to care.
- Collaborate with multidisciplinary care team and expedite sequence of patient's clinical care pathway from E-comms to virtual care, and virtual care to in-person.
- Provide communication and reinforcement of clinical information and serve as a conduit between patient and providers/clinicians to address needs of patients.
- Follow-up and targeted assessment for continuity of care with skin cancer referrals for surgical management with a proceduralist, Mohs or Melanoma Clinic (Cancer Center)
- Assess health literacy and educational needs of patient. Provide clinical education about diagnosis, treatment, side effects, and post- treatment care.Assess and document patient's needs, goals, and preferences and work to ensure such are integrated into treatment and care delivery.
- Bridge gaps in care, and assess/troubleshoot care transitions and barriers to care.
- Work with edit team to optimize the decision and scheduling algorithm. Promote 100% decision tree utilization across all sites..
- Facilitate urgent scheduling overflow appointments from the scheduling center to the clinics
Job Eligibility Requirements:
- Work requires graduation from an accredited BSN or Associate's Degree in Nursing or Nursing Diploma program.
- All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an appropriate BSN program within two years of their start date and to complete the program within five years of their start date.
- Twelve months of appropriate clinical experience is required.
- Must have current or compact RN licensure in the state of North Carolina. BLS required.
Job Hours:
The schedule for this position would be part time, day shift, Monday-Friday with no evenings, weekends, holidays or call.
