JOB SUMMARY:

The Community Partnership Specialist provides support to the Equity, Diversity and Community Engagement Department by identifying and cultivating relationships with community partners, including school districts, surrounding Mountain Park clinics. This role will create a partnership framework for new and existing partners to continue sustaining and embedding Mountain Park in the communities surrounding clinics with a secondary goal of increasing number of new patients.

This person will be a part of a highly collaborative team and will take on or assist with many additional tasks to help achieve department and organization goals.

The Equity, Diversity and Community Engagement Team is dedicated to ensuring Mountain Park is a place where all are welcome and valued and where healthcare is accessible and high-quality regardless of who needs it. The high-energy team works to share the Mountain Park story and the great work that happens at the clinics every day with patients, employees, partners and the surrounding community from a place of equity and inclusion. This is done through videos, social and mainstream media, presentations, trainings, events, community outreach, volunteer engagement and more.

This is a hybrid position, combining remote work, work at the Central Office on Central and Thomas, and work in Mountain Park clinics and surrounding communities.

ESSENTIAL FUNCTIONS:

  • Identifies, cultivates, and manages partnerships and strategic alliances with schools, social service agencies, municipalities, and other community stakeholder groups to develop specific strategies that help eliminate barriers for current and future Mountain Park patients.
  • Co-develops and ensures partnership framework is used to establish and strengthen new and existing partners and collaborations that sustain and embed Mountain Park in the communities we serve.
  • Works with community partners and school districts to increase access to healthcare by identifying and reducing barriers their clients and students face.
  • Partners with the appropriate internal departments and management to develop and coordinate partnership agreements and ensure objectives are carried out.
  • Trains and develops staff’s ability to engage community partners and promotes Mountain Park services at community-facing events by creating best practice framework outlining ways to effectively table, recruit, track and recognize these efforts.
  • Researches and addresses questions, concerns and issues related to current and potential partners and stakeholders.
  • Resolves partnership barriers encountered and determines the need for renegotiation or refresh of partnerships.
  • Develops presentations, conducts meetings, assists in coordination of tabling requests in concert with clinic operations staff and promotes partnerships with the intention of expanding access to care.
  • Adds value and contributes to carrying out other Equity, Diversity and Community Engagement department strategies.
  • Documents patient interactions, partner and clinic events through photos and brief videos.
  • Develops outreach strategy to promote Mountain Park services and engage the community.
  • Assists in developing, planning, promoting and executing community events.
  • Takes a leadership role in community advocacy efforts including voter registration, community forums and primary and general election polling locations.
  • Must be able to embrace differences among people and is able to interact with internal staff as well as external contacts in a culturally competent and respectful manner.
  • Must have a strong tie and strong desire to make a difference in medically underserved and historically disenfranchised communities.
  • Must be able to work flexible hours which may include evenings and weekends.
  • Must be comfortable and accountable for working remotely and traveling to multiple clinic sites.
  • Performs all other duties as assigned.

ADDITIONAL RESPONSIBILITIES:

  • Promotes positive patient/guest relations in accordance with Mountain Park policies, providing a high level of quality in personal attention and service to patients and visitors.
  • Complies with Mountain Park Safety Policies and Procedures. Ensures compliance with governmental licensing and regulatory requirements where applicable. Conducts himself/herself in a manner consistent with the Mission Statement and Values of Mountain Park.
  • Observes and respects the confidentiality of information in regard to patients, visitors and co-workers employees including salary information, if exposed to any of the above referenced information in the course of his/her job function.
  • Demonstrates an understanding of facility organizational structure by utilization of appropriate channels of communication. Maintain consistent, timely communication regarding all facets of departmental activities with his/her co-workers, therefore, demonstrating good customer service skills with both external and internal customers.

POSITION QUALIFICATIONS:

Minimum Qualifications:

  • Bachelor’s degree in a relevant field including communications, public relations, public health, public administration, business administration, health care administration or health-related studies is preferred OR equivalent job experience.

Preferred Qualifications:

  • Five years of work experience with a minimum of two years working with nonprofit organizations preferred.
  • Ideal candidate will have experience working with diverse communities, staff coordination, establishing strategic alliances, business development and project management.
