Adapt Integrated Health Care is looking for a Case Manager to support their SUD services!

The SUD Case Manager provides case management, needs assessments, and care coordination to substance abuse and co-occurring clients, assisting those clients in reaching self-sufficiency.
Primary Responsibilities:
- Collaborates with the client on a needs assessment to identify areas of focus
- Assists with recovery plan development and implementation, coordinating with the primary counselor and other key staff
- Investigates resources and provides referrals to appropriate community agencies
- Integrates client supports (family, social, community) to help remove barriers for clients
- Coordinates referrals, within the ASAM level of care, to residential facilities to affect a smooth transition between services
- Documents in a timely and appropriate manner each intervention and coordinates information with the client’s support team
- Attends staff meetings and development trainings as scheduled
- Participates in clinical supervision, including QA review of documentation and personal goal tracking highlighted in yearly evaluation
Qualifications
Required Qualifications:
- High School Diploma or equivalent required.
- Completed MHACBO minimum training and experience requirements for CADC I or higher.
