Job Summary
As a Care Manager RN 2, play a vital role in patient care. You assess and coordinate care using guidelines to manage services effectively. You are a care provider, case manager, mentor aiming for quality outcomes by managing resources efficiently.

Setting: Case Management
Schedule: FT Mon-Fri
What You'll Do
- You will manage care plans, working with our health care team and using medical best practices.
- You will establish patient care management plans. This includes crafting interventions, establishing treatment goals, and setting self-management goals.
- Your role is to assess the needs of the patient and family, including healthcare, education, and psychosocial needs.
- You will help patients manage health conditions, educate through visits, calls, and guide adherence to care plans.
- You will research and recommend resources for various patient needs, medical and non-medical. Your role includes coordinating services between programs smoothly.
- You will earnestly partake in identifying and problem-solving care coordination inefficiencies, thereby facilitating optimal care.
- You will mediate between hospitals, primary caregivers, specialists, and post-acute partners in your role.
- You'll guide staff, improve processes, and address questions as a skilled in care management workflows.
- Your constant effort towards staff retention, which includes responding promptly to their concerns, will be highly valuable.
- You will oversee new staff orientation, support preceptors and novices, evaluate performance, and provide feedback to managers. Participating in staff interviews is important.

- A specialty certification would be advantageous.
- Being knowledgeable about care management, resource allocation, and utilization management will be key.
- You should possess skills in assessing and managing patient care.
- Effective verbal and written communication skills are essential.
- You should have strategic, critical thinking capabilities for problem-solving and addressing concerns.
- You will need the ability to monitor and record patient progress against a care plan.
- You should be able to assist patients in accessing community resources.
- You must cooperate with everyone in medical field, inside and outside organization, for success.
- You should be able to assess, adapt to, and respond calmly to a fluctuating and crisis-prone environment.
- You'll need to maintain confidentiality regarding all aspects of patient info, conforming to all pertinent policies and regulations.

Qualifications
- Associate Degree
- A minimum of (3) three years of relevant work experience
- Must possess a current and valid Registered Nurse license
